To request conciliation, arbitration or adjudication from the Ontario Police Arbitration and Adjudication Commission, please fill out and print the appropriate form below. The files are in .pdf. You may need to download Adobe Acrobat Reader to view the PDF forms on this page.

Instructions for Completing an Application
Mandatory Content
- Summary of Allegations: All applications must include a summary of the allegations, referencing the applicable sections of the Community Safety and Policing Act (CSPA).
Prohibited Content
- No Evidence or Sensitive Information
- Applications may not include evidence, exhibits, or argument.
- Names of witnesses and complainants must be anonymized (e.g., use initials).
- Do not include confidential information such as cell phone numbers, dates of birth, or addresses.
- Do not include medical information
- No Appendices
- Do not attach appendices. The summary box on the form is expandable.
- Supporting documents (e.g., correspondence, collective agreements, witness statements, occurrence reports, or appended summaries) will not be accepted.
- Any additional information should be provided directly to the appointed adjudicator. OPAAC will not forward appended information.
Purpose of These Instructions
These guidelines protect privacy and confidentiality and ensure both parties have equal opportunity to provide evidence and other information during the pre-hearing and hearing processes.
Applications that do not comply with these guidelines will be rejected and returned to the applicant for correction.
Conciliation and Arbitration Matters:
- Please refer to the applicable collective agreement for your application and submit a copy in a separate email if one has not already been filed with OPAAC.
Additional Notes
- Please download the form and save your final application before submitting to OPAAC.
- The space provided on the application form expands to allow applicants to describe the reasons for the application.

Information provided in your application, including correspondence to the Ontario Police Arbitration and Adjudication Commission (OPAAC), is subject to the Freedom of Information and Protection of Privacy Act (FIPPA). It may be disclosed in response to a formal FOI request under Section 11 (Obligation to disclose) of the Act.

If this matter is an OCPC transitional appeal referred to OPAAC pursuant to section 216 of the Community Safety and Policing Act, 2019, you must also forward a copy of your completed application, notice of appeal, and the hearing officer’s decision that is the subject of the appeal to the Ministry of the Solicitor General, Legal Services Branch Mailbox: legalservicesgeneralmailbox@ontario.ca.
